Insurance Coverage for 3D Areola Tattooing

At The Medical Tattoo, we specialize in 3D areola and nipple tattooing as the final stage of breast reconstruction after mastectomy.

Under a federal law called the Women’s Health and Cancer Rights Act (WHCRA), most health insurance plans that cover mastectomy are required to also cover all stages of breast reconstruction, including areola and nipple restoration. This means that for breast cancer patients, 3D areola tattooing is considered medically necessary reconstruction, not a cosmetic procedure.

How insurance coverage works

  1. Your surgeon provides a prescriptionYour plastic surgeon or breast surgeon writes a brief Letter of Medical Necessity stating that 3D areola tattooing is part of your breast reconstruction following mastectomy.
  2. You receive your treatment at The Medical TattooYour 3D areola tattoo is performed by our medical tattoo specialists.
  3. We provide your paperworkAt the time of your first treatment, we provide you with:
    • A Superbill (the insurance billing form)
    • A detailed receipt
  4. You submit to your insurance
    You submit the Superbill, receipt, and your surgeon’s Letter of Medical Necessity to your insurance company for reimbursement.

  • Superbill from The Medical Tattoo
  • Paid receipt
  • Breast cancer and mastectomy diagnosis codes from your doctor that will go on your superbill provided by The Medical Tattoo

Important note

Every insurance plan is different. Some plans require pre-authorization before treatment, so we recommend calling your insurance provider in advance to confirm what they need. You can find my Superbill under “Forms” on this website to use for a pre-auth if necessary.
